What TEAM UP Certification Means

Certified practices have successfully implemented the TEAM UP model. By earning certification, a practice has:

Integrated Care Team

Built a fully integrated pediatric care team that includes behavioral health clinicians, community health workers, and primary care providers working collaboratively to address patients’ developmental and behavioral health needs.

Screening

Adopted universal social, developmental, and behavioral health screening as a routine part of well-childcare, enabling early identification of needs.

Workflows

Established warm handoff workflows and timely access to developmental and behavioral health services for children and families within the pediatric medical home.

Evidence-informed Interventions

Delivered evidence-informed, family-centered interventions tailored to the developmental and behavioral needs of children from birth through young adulthood.

Learning Community

Participated in TEAM UP’s Learning Community to strengthen team roles, stabilize workflows, and standardize care delivery. 

Data-driven Quality Improvement

Used data to guide implementation, measure fidelity, and continuously improve care, with progress tracked through the TEAM UP evaluation reports. 

Certification affirms that a practice has met the full set of expectations for high-quality, team-based integrated behavioral health care in pediatrics, and reflects their deep commitment to delivering comprehensive, integrated behavioral health services that are responsive to the needs of children and families.
